The AHCCCS URL, www.azahcccs.gov/regulations/lawsregulations.aspx, must appear on all NOA's, even if the only legal basis is not on the AHCCCS website.

A NOA issued for any termination of service should reflect an effective date at least 10 days later than the date of the NOA. This means that if a NOA is issued March 7, 2011, the effective date and the service continuation date is Thursday March 17, 2011. However, if the last day is on a weekend or holiday, the time period is extended to the next business day.
- No NOA is required when a service is declined.
- The NOA must state the reason for the requested service. (What is the problem or condition that the requested service is supposed to address?) This statement of the reason for the requested service must meet the 6.5 grade level readability requirement. The minimum notice requirement for service termination when an person determined to have a serious mental illness (SMI) is receiving a Title 19 funded service is 10 days.
